You can submit a complaint regarding issues under OPI's responsibility by filling in the fields of the following form.
You are also provided with a similar form to submit a Request.
Please be advised that you can monitor the progress of your complaint at all times; to do so, enter your contact details together with the reference number automatically generated once your complaint was successfully submitted. If you are a registered user your contact details have already been stored during your registration process and are not required.
Please also note that the description of your complaint should be no more than 1500 characters.
